A onetime $50 processing is required to enroll via the checking or savings account you wish to use and will be processed after filing out this application. Your VIP monthly membership starts the first day of month after the month you signed up (unless you sign up exactly on the first of that month), and processes the first of each month thereafter. Your member discounts and benefits renew on that date as well. Your membership fee is deducted from your checking or savings institution automatically each month. Your skin is a long-term investment. The minimum membership term is 12 months.In order to cancel a membership after the 12 month minimum term, a 30 day notice is required. To cancel a membership before the 12 month term is complete, a $500 fee is applied. Our typical member stays with us for several years for maximum benefit to the member. Banking your monthly investment can be saved for as many months as you wish until you come in to redeem the saved monies. Banking your monthly investment can be saved up until the end of your one year commitment and it may carried over into the next year by only three months. If choosing to use your customized exfoliation treatment/chemical peel – it must be used that month as it is non transferable and unable to carry over – no exceptions. This is a member only benefit program and cannot be transferred to friends, family, or children in any manner. Our 48-hour cancelation policy still applies for all scheduled appointments and a $50 fee will be charged if this policy is not followed. After more than 3 less than 48 hour cancellations/no shows in a one year period, this will require a $100 deposit via credit card to book any future appointments. If funds are insufficient in your banking checking/savings account – you will be responsible to refund us any fees charged to us for insufficient funds. If this happens two times or more during your participation in the VIP program, you will be removed from it and no longer allowed to participate in it for the foreseeable future. Treatment prices are subject to change at any time and without notice. This may be due to unforeseen variables such as supply costs, demand and supply factors, market values, etc. Contracted discounts in the VIP program are not subject to change and are locked in for a 1 year period from the original contract begin date. *Dysport discount is exchanged out at the same percent discount as Botox ($1.25 off per unit) which equates to a $0.42 discount per unit with Dysport. **Filler discount is applied to an office visit. The multiple syringe discount is for multiple syringes used in the same office visit only.
